In this role, you will support young people leaving care to have a quality plan that outlines supports, referrals and prepares them for adulthood, as well as support the delivery of the plan.
What you'll do
The role of Caseworker – After Care will have a focus on:
- Ensuring all young people leaving statutory out of home care have an approved After Care Plan and provide support should these aftercare plans require changes
- Active follow up with young people after they turn 18 to check they are safe and well, and to identify if they are experiencing any additional needs, or are at risk of homelessness
- Support young people with their transition to adult supports and services, providing advocacy, whilst also building upon their capacity to access and advocate with services.
- Respond to the young person if they seek support or information
- Being a key contact for all young people who have exited statutory out of home care and Continue to support their connection to family and culture.
- Develop and maintain strong partnerships with key stakeholders, including adult community health services, NDIS, Public Guardian, and various accommodation services.
- Providing an out of home care quality assurance and audit function to improve the provision of casework services for children and young people aged 15+
- Delivering and co-facilitating training to support Caseworkers in their casework with young people leaving statutory out of home care

What we're looking for
- Bachelor Degree in social work, social science, community welfare, youth work or a related discipline
- A valid Australian Driver’s Licence
- Minimum 2 years’ experience as a Caseworker
- A paid working with children clearance number
